Job Title: Family Med Physician – MD or DO

Summary: Provides comprehensive high-quality patient care services for all patients regardless of insurance status or ability to pay on continuing basis adhering to the Patient-Centered Medical Home care delivery model by performing the following duties.
Essential Duties and Responsibilities: include the following, other duties may be assigned.
Reviews patient file/record, including allergies, problems, medications, and immunization status.
Elicits and records information about a patient’s medical history.
Examines patients for symptoms or physical information.
Orders or executes various tests, analyses, and diagnostic images to provide information on patient's condition.
Analyzes reports and findings of tests and examinations, and diagnoses condition of patient.
Administers or prescribes treatments.
Determines and prescribes medication, dosage, and schedule given the patient's condition and allergies.
Discusses any possible medication/immunization side effects with patient.
Prescribes and administers vaccinations to immunize patients from communicable diseases.
Promotes health education by advising patients about diet, hygiene, and methods for prevention of disease.
Performs surgical procedures commensurate with surgical competency.
Refers patients to medical specialist for consultant services when necessary.
Documents the patient's visit including medical history, physical exam, diagnoses, and plan of action.
Follows up with patient regarding progress in high-risk/emergency cases.
Conducts physical examinations to provide information needed for admission to school, consideration for jobs, or eligibility for insurance coverage.
Follows Center protocols for meeting Uniform Data System (UDS) standards.
Observe and refer patients for dental and/or behavioral health care as deemed appropriate based on the Center’s integrated care model guideline.
Participates in Patient-Centered Medical Home team morning huddles and provider meetings.
Participates in assigned quality improvement initiatives.
Participates in community health outreach activities.
Other tasks as indicated by CMO or CEO.
Certificates, Licenses, Registrations:
Current VA medical license, DEA and Controlled Substance, current driver's license.
Other Requirements:
Travel and or work at a Center other than primary assigned Center may be required. Occasional and local travel within and outside of the state may be required.
Physical Demands:
While performing the duties of this Job, the employee is regularly required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to sit. The employee is occasionally required to stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
Work Environment:
While performing the duties of this Job, the employee is occasionally exposed to moving mechanical parts, fumes or airborne particles and risk of radiation. The noise level in the work environment is usually moderate.
