Responsible for serving as the first reliable point of contact for Patients, Families and Providers. Principal Accountabilities: 1. Serves as patient and family advocate. 2. Coordinates the scheduling of appointments to expedite the plan of care and to promote continuity of care. 3. Schedules, re-schedules, and cancels appointments for the appropriate outpatient service.4. Assists in education of the patient/family to understand disease process and care map. 5.
