Case Manager/Social Worker, MSW - Patient Case Management
OverviewThe Case Manager, Social Worker is assigned to specific clinical care units and/or specific patient populations to provide services to patients and families whose medical, psychological and/or social situation impacts compliance with the physician care plan, the provision of appropriate health care services for the patient, and/or timely discharge. Coordinates the assistance necessary to meet patient specific needs relative to placement, basic living resources, home health resources, counseling, and post-hospital support in order to optimize outcomes. Works collaboratively with the Nurse Case Manager and other patient care team members. Serves as an advocate to ensure that the patient's physical, psychological, and social needs are identified and fulfilled
