JOB SUMMARYThe Clinical Liaison initiates and develops relationships with current and potential referral sources and evaluates patients for post-acute care services. The liaison performs pre-admission assessments to determine patient candidacy for admission to the inpatient rehabilitation center. The liaison works collaboratively with physician and nurse leaders to ensure compliance with Medicare guidelines and proper documentation both on pre-admission and throughout the patient's rehab stay.ESSENTIAL FUNCTIONS OF THE ROLECompletes preadmission assessments timely, thoroughly, and with sufficient detail to justify a rehab admission.Coordinates admission process with referral source, patient/family and rehab team through effective communicati
