Job SummaryFull Time Day 8am - 4:30pm Monday - Friday; REQUIRES: High School Diploma or GED and 2 years related experience; prior neurology or patient navigation experience preferredJob DutiesCoordinates patient care and maximizes patient satisfaction and experience.  Partners with care providers to serve as a liaison to ensure effective communication with patients.Provides discharge information.Partners with providers to ensure a smooth transition at discharge to home or other facilities as required.Ensures additional resources are made available for the patient as needed.Effectively collaborates with other healthcare providers and non-Geisinger employees in coordinating and implementing the patients plan of care.
