The role you’ll contribute:
Provides vocational nursing services under the supervision of an RN, to patients in their homes.
The value you’ll bring to the team:
o Performs nursing skills safely and competently according to the plan of care.
o Provides accurate and complete documentation the same day of the visit.
o Schedules visits to facilitate patient needs and completes assigned schedule in a timely manner.
o Plans patient care in consultation with Registered nurse.
o Assists with patient/caregiver teaching.
o Demonstrates flexibility in response to unexpected changes and the ability to adjust priorities and use resources.
o Maintains open communication with RN Case Coordinator and RN Supervisor.
o Demonstrates suitable time management skills.
o Participates in case conferences and staff meetings.
o Participates in agency Performance Improvement activities.
o Provides orientation to newly hired LPNs and hosts student LPNs.
The expertise and experiences you’ll need to succeed:
· Minimum qualifications:
o Licensed Practical Nurse licensed in the state of Florida
o Basic Life Support certification
o Valid Florida driver’s license, safe driving record and reliable transportation
o Minimum 1 year of nursing experience as a LPN
· Education and Experience Preferred:
o LPN IV Certification as required for Florida Board of Nursing (Certification of completion for 30-hour course)
o Home care experience
