2 years NP experience in Primary Care required. Community Health patient population primarily adults. Monday through Friday 8 am to 5 pm. FT or min of 20 hours PT with 4 hour blocks. Required 5 state licenses with a strong preference for CT and IL. CT takes 5 weeks for an NP license and is a compact state. Provider will need on PC for remote work along with a dedicated work space for telehealth visits.. Proficiency with technology and confidence working solo is essential. Duties: - Perform health histories and assessments - Diagnose and treat acute and chronic illnesses - Prescribe medications and other treatments - Provide counseling and education to patients on health promotion and disease prevention - Collaborate with physicians and other healthcare professionals to develop and implement patient care plans - Maintain accurate medical documentation and patient records Experience: To be successful in this role, you should have the following skills and qualifications: - 5 current valid Nurse Practitioner licenses in ME,NH,VT,MA,RI,CT,IL,IN,TX,UT,NV,FL,GA,CA,NY,NJ,CA,WA - Proficiency in medical documentation and varying EMR systems. - Knowledge of sexual health services including PrEP, doxyPEP, Hepatitis C treatment and HIV care.
