Job SummaryThe Clinical Nurse Care Manager (CNCM I) position supports the healthcare team in transitional care by being a liaison between patients, their families, doctors, and health care providers. CNCM I facilitates transitional care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction. In addition, the CNCM I helps to drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, process improvements, patient satisfaction). CNCM I is accountable for patient's caseload and plans effectively in order to meet patient needs across the appropriately deemed continuum of care, provide family support, manage the length of stay, and promote
