A Brief Overview
Identifies issues and works to resolve clinical and utilization issues affecting the service line. Integrates the quality metrics and performance improvement programs within the clinical and financial aspects of the service line. Responsible for managing, planning and coordination of quality to include patient/family satisfaction, audits for compliance, and clinical documentation review. Participates in the analysis of data regarding trends in utilization, outcomes and quality. Participates in the educational activities within the service line.
What you will do
- In collaboration with the Director of Home Care & Quality Team, take lead in discussing/determining the strategic approach to achieve compliance to the regulatory requirements for Home Care.
- Review clinical and financial data to conduct a quantitative analysis on healthcare and quality issues such as utilization, cost and patient experience.
- Review OASIS assessments for accuracy, compliance and correlation to the Plan of Care.
- Analyze third-party/contractual reports to identify trends and implement process improvements if applicable.
- Analyze data and summarize findings to facilitate the development of education and quality improvement activities to support the service line and maintain the activities as appropriate.
- Take Lead in staff education that is related to OASIS metrics, utilization and financial impact.
- Collaborates among the departments within the service line to continually problem solve, seek improvement in processes that affect the quality and delivery of care and assists in the implementation of processes to improve service.
- Establishes mechanisms to identify issues and track the corrective action.
- Ensures the service line’s performance improvement (PI) program is comprehensive and integrated and compliant with all regulatory bodies, licensing authorities and agency policies.
- Develops and maintains effective communication with all staff to facilitate continuous readiness and an innovative approach to problem solving.
- Responsible for the management and improvement of Star Ratings.
- Responsible for patient safety data and the identification process changes to improve results.
- Responsible for the management and oversight of Home Care Policies in collaboration with Quality Team.
- Manages the review and aggregation of OBQI/OBQM data.
- Manages Quality Team scheduling, time off requests and coverage.
- Manages quality improvement (QI) activities and reports results quarterly to Quality Team.
- Participates in Administrative On Call Duties
- Accepts and completes additional responsibilities as directed by management.
- ct State Licensure Current RN licensure in the state of New Jersey upon hire required

Experience Qualifications
- 3-5 years of experience in community health/home care setting preferred
Knowledge, Skills, and Abilities
- Knowledge of federal and state standards applicable to homecare
- Knowledge of governmental and other regulatory standards, requirements, and guidelines related to quality improvement
- Strong working knowledge of ongoing monitoring of care delivery in homecare and quality management principles, tools and techniques
