The Patient Scheduler Coordinator is responsible for coordinating scheduling of all field employees; provides adequate staffing.
The value you’ll bring to the team:
Manages the schedule for all patients. Edits schedule for employees calling in sick, ensuring patients are reassigned timely. Initiates infection control forms as needed and uploads as 'Employee Infection Report' in HR Console.
Receives second calls from the field staff (if LPN) during times the clinical supervisors are not available such as during lunch time, weekly case conferences, and other busy times. Refers clinical questions to Manager or Director as necessary.
Maintains the patient hospitalization log, including entering coordination notes in electronic medical record, and updating daily for morning huddle. Provides appropriate transfer summary and medication list to the receiving inpatient facility.
Completes requested schedule as task appears in workflow according to agency scheduling guidelines.
Completes requested schedules for all add-ons and applicable orders:
Schedule discharge visit / OASIS Collection or recert visit following case conference when task appears on action screen.
After TIF OASIS collection deletes remaining schedule.
Reschedule declined or missed (if appropriate) visits.
Process reassigned and rescheduled visits.
Ensure Supervisory Visits are scheduled according to agency protocol and regulatory mandates.
Qualifications
The expertise and experiences you’ll need to succeed:
REQUIRED:
High school diploma or GED.
One year of related experience in medical office.
Maintain a current, valid Florida Driver's License in good standing with a safe driving record and a reliable vehicle.
Maintain valid personal automobile insurance.
PREFERRED:
Home health agency office experience
