Job Summary:
We are seeking a CLINICAL NURSE II in Lakeview Outpatient Behavioral Health Clinic located in Durham to be an integral member of the healthcare team and provide direct and indirect care to patients.
Job Duties and Responsibilities:
Plan, provide, supervise and document professional nursing care utilizing the nursing process for both pediatric and adult patients in accordance with physician orders and established policies and procedures. Use professional nursing judgment to individualize the plan of care based on assessment of the patient's baseline needs and response to care.
Delegate tasks and supervise the activities of other licensed and unlicensed care providers.
Assist other nursing personnel in the delivery of nursing care and act as team leader or charge nurse for a group of patients or an entire unit as assigned.
Rooming patients which includes vital signs, current medication and pharmacy review, falls assessment, etc. and notifies provider of any concerns.
Completes all of the appropriate steps for patient or pharmacy requested refills including assessing electronic chart for appropriateness, verifying correct pharmacy and queueing up for provider once determined. Calling pharmacies to work through issues at every level of the medication ordering, filling, refilling and insurance needs.
Completing Medication Prior Authorizations-gathering clinical information based in the individual questions presented for each PA, conferring with provider to answer specific questions not otherwise addressed in the chart. Working with provider on next-steps. Notifying patient/parent/provider of outcome and documenting appropriately. May need to re-submit original or assist in appeal process if PA has negative outcome.
Working with insurance companies including NC Medicaid for any needs that may arise in the medication ordering process.
Responding to all calls that are routed to the Nursing line. These may include medication questions, safety assessments, coordinating care with internal and external resources (providers, clinics, schools, DSS, etc.) scheduling, some intake and initial triage, patient and family education, etc.
Providing appropriate screening tools as requested by the provider.
Complete or assist with patient specific notes and forms as requested by the patient.
Managing all patient questions/concerns/refill requests and staff communication in EPIC/MyChart.
Work closely with the multidisciplinary team to address medical, psychiatric, psychosocial and other concerns for patients and their families.
Placing lab orders as directed by providers.
Collecting labs either by venipuncture or urine specimen and handling appropriately to ensure they are tested and resulted on in a timely matter. Documenting as appropriate including charges and charge capture
Performing EKG's when ordered.
Assisting with any on-site medical and behavioral health emergencies including being a point person during the IVC process.
Gain familiarity in the services provided at Duke and in the community and act as a subject matter expert in these resources, access to, and limitations of access to these resources including insurance knowledge.
Perform other related duties incidental to the work described herein.
Job Eligibility Requirements:
Work requires graduation from an accredited BSN or Associate's Degree in Nursing or Nursing Diploma program.
All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an appropriate BSN program within two years of their start date and to complete the program within seven years of their start date.
Twelve months of appropriate clinical experience is required.
Must have current or compact RN licensure in the state of North Carolina. BLS required.
Job Schedule:
The schedule for this position is 8am-5pm Monday-Friday, with no weekends or holidays
