ChristianaCare is currently seeking a full-time Licensed Social Worker to support our primary care practice located in Elkton, MD.
The Social Worker works as a valued member of our primary care team to addresses potential barriers to care by facilitating and coordinating connection to available resources to ensure patients can access care.
This position will assist patients and their families in to identify social, emotional, and environmental needs and to connect them with available resources/ services.
Work Schedule:
Monday- Friday: 8am -4:30pm
This position is a hybrid blend of remote/virtual and in person work.

Key Responsibilities:
Participates as a member of the practice care team, focused on addressing financial barriers to care, care coordination, and connecting to available community resources to address social care needs of patients.
Provides information about resources and options available in the community and coordinates service delivery.
Interprets patient/family needs and provides information concerning the availability and limitations of resources.
Educates and addresses concerns with service delivery including service gaps and access issues.

Qualifications:
Bachelor’s degree in social work or related field from an accredited college/university required.
Licensed BSW required.
