JOB SUMMARY
Assists the nursing staff and patients with basic care needs, implementing orders, and evaluating care for various patients within the facility. Reports directly to the charge nurse and Clinical Director of the unit. This position works in conjunction with other members of nursing service, ancillary services, physicians, patients, and families. It collaborates with various members of the healthcare team to ensure quality of care across the continuum. Requires the ability to be self-directed and quality focused. Interpersonal communication skills are a necessary function of this position.

Vanderbilt Wilson County Hospital (VWCH) has received the American Heart Association's Get With The Guidelines-Stroke Silver quality achievement award for its commitment to ensuring stroke patients receive the most appropriate treatment according to nationally recognized, research-based guidelines. This has ultimately saved more lives and reduced disability.

VWCH also received the American Heart Association's Target: Type 2 Diabetes(tm) Honor Roll award. Target: Type 2 Diabetes aims to ensure patients with Type 2 diabetes, who might be at higher risk for complications, receive the most up-to-date, evidence-based care when hospitalized due to stroke.

Schedule:

Full-Time Nights: 6:45 pm - 7:15 am

Rotating weekends and holidays

Stepdown: This rewarding position on our 14-bed Stepdown unit will care for adult patients 18 and older who are being treated for medical conditions requiring hemodynamic stability and monitoring every 2 hours or less frequently. The patients on our unit may require limited titrated drips. Our patients are unique because they require close clinical monitoring, technical observation, and rapid interventions that are generally beyond the usual capabilities of the general patient care units.

Qualifications:

Mandatory: High School Diploma or GED (or equivalent experience) and one of the three below:

• Certification: Graduation from a Nursing Assistant or Aide Training Program, (the certification is not mandatory, but a certificate of completion is necessary) or

• Experience: Minimum of six months of previous experience as a care partner or nursing assistant/technician or

• Education: Current enrollment in a program of professional nursing LPN or RN and successful completion of the first semester of nursing school to include nursing fundamentals

KEY RESPONSIBILITIES
* Obtains vital signs, Intake and Outputs, and daily weights
* Reports any abnormal findings to the nurse and/or charge nurse
* Assists patients with activities of daily living, bathing, and toileting
* May transcribe physician orders, answers patient call lights and relays messages to staff in a timely manner
* May assist physicians with obtaining needed information on patients
* Adheres to practices and policies outlined to ensure patient safety and follows infection control standards

TECHNICAL CAPABILITIES
* Environment of Care (Fundamental Awareness): Has limited experience or training working with set up and maintenance of patient exam rooms, supplies and equipment.
* Care Partner Patient Documentation (Fundamental Awareness): Has limited experience or training working with clinical applications for patient documentation.
* Planning & Organizing (Fundamental Awareness): Demonstrates ability to anticipate basic resource needs, identify discrete tasks, set priorities, define dependencies, schedule activities, meet deadlines, and organize work for own assignments and responsibilities. Appropriately matches equipment, and capital resources to task demands. Prepares and presents clear and accurate status reports on projects and assignments.
