The VAs Eastern Colorado Health Care System, PFC James Dunn VA Clinic in Pueblo, CO, is seeking full-time Family Practice or Internal Medicine trained physicians to join our Primary Care team. The Primary Care Provider (PCP) will function as a highly skilled, independent care provider in the management of patients and will be part of an interdisciplinary care team with access to state-of-the-art medicine and some of the most advanced medical technologies available.
Our Primary Care Provider functions as a team provider in a shared workflow with a multidisciplinary Patient-Aligned Care Team (PACT - a medical home model). The PACT team model consists of an MD (or PA/NP), RN, LPN, and Medical Support Assistant. Providing leadership to the team including shared delegation of appropriate care and care processes to appropriate team members. You will be assigned to provide comprehensive, collaborative same-day and continuity care to an assigned panel of Veterans via face-to-face or telehealth modalities utilizing all available tools, such as registries, to enable effective population health management. PACT Providers are expected to demonstrate excellence in customer service via compassionate and informed interpersonal relationships with Veterans, family members, and staff.
The Primary Care Provider experience should include:
comfort with chronic disease management and geriatric patients
comfort in providing pain management
comfort with addiction treatment
comfort in delivering mental health treatment to our Veteran population
comfort in providing women’s health care
Other duties:
Participating in team performance improvement and sustainment activities to optimize team efficiency and care delivery to patients.
Providing health education and health coaching on wellness, disease prevention, chronic care management, and self-management skills to patients and personal support persons, commensurate with the documented expertise or professional training of the PACT member.
Reviewing available clinical and performance data with the team, focusing on continuous improvement of critical team processes.
Ensuring the patient has same-day access for face-to-face and telephone care visits during regular clinic hours.
Work Schedule: Monday-Friday, 40hrs/wk (start/end time flexible)

Candidates must be: 1) US Citizen (Non-Citizen May Be Considered IAW VA Policy), 2) Residency trained in Family or Internal Medicine, and 3) Have a full, unrestricted medical license in any State Preferred Experience: Board Certified in Family or Internal Medicine; VA experience a plus, three or more years experience in Primary Care setting
