The role you’ll contribute:
The Registered Nurse (RN) Readmission Coordinator, in partnership with patients, caregivers, providers, nursing and the multidisciplinary healthcare team, utilizes professional skills to reduce unnecessary readmissions. Coaches patients and families in disease self-management and establishes plans for safe and effective transfers in the movement of patients across the continuum of care. Promotes and evaluates the effective utilization of resources using current clinical knowledge, awareness of community services to achieve optimal clinical and resource outcomes. Patients will be identified by various methods including collaborating with the Transition of Care Team members and Providers. Transitional management will be provided to the patients who have the highest risk for readmission and strategies will be appropriate to that level. The RN Readmission Coordinator works as part of an interdisciplinary team including the readmission prevention physician leader, medical providers, transition of care team, nursing leadership, nursing staff, pharmacists, respiratory coordinators, and home care liaisons. The scope consists of inpatient, outpatient, and emergency room patients.
The value you’ll bring to the team:
·Review reports on Current Core Measure Hospitalizations and Re-admissions daily ·Coordinates the Readmission Prevention Committee Meeting bi-weekly and: ·Identifies 3-5 recent readmission cases and reviews for any possible opportunities for improvement ·Creates and sends out meeting Agenda ·Completes meeting Minutes ·Collaborates with multidisciplinary team including Attending physician, Case Management, Cardiopulmonary Rehab, Respiratory Therapy, Advanced Heart Failure Team, Heart Failure Coordinator, Chest Pain Coordinator, Care360 Team when applicable with goal to reduce readmission rates as it relates to the CMS targeted diagnosis ·Performs all other duties as assigned ·Participates in In-patient visits and post-discharge phone calls: ·Utilizes Qlik View report to identify patients who are hospitalized with Core Measure diagnoses of Heart Failure, Acute Myocardial Infarction, Coronary Artery Bypass Graft procedure, Pneumonia, Chronic Obstructive Pulmonary Disease, Sepsis secondary to Pneumonia, Stroke, Total Knee Arthroplasty, Total Hip Arthroplasty o Visits patients in the hospital and provides education on disease process and medications
Qualifications
The expertise and experiences you’ll need to succeed:
·Associate in Nursing and 3 years experience of acute care clinical experience ·Basic Life Support - CPR Cert ·Current license as a Registered Nurse in the State of Florida ·Current Basic Life Support certification.
