Job Summary
Establishes, coordinates and provides clinical pharmacy services within the hospital. Functions in the role of consultant and educator of drug therapy to health care workers. This position will care for patients in the neonate, child, adolescent, adult, and geriatric age groups. Employee performs clinical duties in accordance with population specific guidelines and adheres to National Patient Safety Goals as outlined in policy and procedures.

Essential Tasks and Responsibilities
Serves as a resource for medical staff, nursing, ancillary professionals and the community regarding pharmacy service in their designated area of practice to ensure quality patient care.
Provides cost-effective drug therapy recommendations to physicians in a professional manner.
Responds to drug information requests in a thorough and timely manner for all healthcare professionals and patients.
Works with the nursing staff on special projects to increase safety and quality of care to patients.
Coordinates the development and implementation of patient education for patients in their specialized area of practice including counseling patients about medications to ensure effective drug therapy and assisting in coordination of medical services for patients with financial needs.
Actively participates in any clinical trials effecting their specialty area of practice by evaluating, interpreting, and assisting in implementation of clinical trials, coordinating and assisting with patient dose modifications, side effects monitoring, and reporting of adverse drug reactions during clinical trials and by insuring that investigational drugs are stored, dispensed and destroyed per protocol.
Identifies and provides for learning needs of patients, families and members of the health care team through formal and informal means to facilitate quality customer service.
Educates patients and families about medication when requested.
Provides educational in-services to pharmacy staff, nursing staff and other healthcare professionals on topics when requested.
Serves as a preceptor to pharmacy students.
Participates in the staff pharmacist Staff Development Program.
Assists in the monitoring and reporting of programs affecting the Pharmacy and other departments to maintain a quality clinical program.
Assures JCAHO standards are met by completing the drug use evaluations, adverse drug reaction reports, and medication error reports.
Actively participates in departmental continuous quality improvement and quality assurance projects.
Assists in preparing for the Pharmacy and Therapeutics Committee meetings by gathering and coordinating information for discussion.
Assists pharmacists in their clinical involvement of therapeutic drug monitoring and clinical skills in their specialized area of practice. Assists in cost analysis of clinical pharmacy services and formulary decisions and takes an active role in the Pharmacy and Therapeutics Committee and other medical staff committees as appropriate.
Accepts special assignments for development, review, analysis, and recommendations.
Specific responsibilities of Antimicrobial Stewardship Specialist: (1) Collect and report intervention data for Antimicrobial Stewardship Team; (2) Conduct routine pharmacoeconomic assessments of the Antimicrobial Stewardship program across the health system; (3) Coordinate agenda for Antimicrobial Subcommittee of P&T
