Job Summary:
JOB SUMMARY
This full-time nurse practitioner will spend her/his clinical time in the Pediatric Primary Care Clinic. Approximately half of the job will be in providing pediatric primary care, while the other half will be in providing nutritional counseling to children with obesity. Specific job responsibilities include: history taking, physical exams, and assessments of newborns to children age 21 using a medical home model which supports both well child exams and acute visits. The NP will be expected to: 1) order age-appropriate immunizations, assess physical growth, and perform developmental assessments, screening tests and well child exams in compliance with AAP, Bright Futures, and EPDST guidelines; 2) refer patients to appropriate specialty services; 3) order appropriate labs and imaging as indicated; 4) treat minor acute illnesses and refer to appropriate specialties in accordance with physician-developed and guided protocols; 5) work to improve patient flow through clinic by seeing lower acuity patients efficiently; 6) convert acute visits to EPSDT/checkup visits when possible; 7) communicate effectively with parents and caregivers regarding assessments, treatments, and management; and 8) collaborate and communicate in a professional manner with various members of our medical home team including residents, attendings, social workers, case managers, psychologists and staff. Because of the high rate of childhood obesity among our patients, we are also seeking a practitioner with interest in supporting clinic-based nutritional counseling. Our clinic has recently been awarded a grant to support embedded obesity management. While experience with nutritional counseling is preferred, an interested practitioner will be trained by our team of experienced dieticians to deliver evidence-based nutritional interventions in our clinic setting.

DESCRIPTION
The Division cares for a large, diverse patient population, and we seek to provide care consistent with a medical home model. We are a certified Patient-Centered Medical Home (PCMH) and are working to improve continuity for patients with their primary provider, and this position will support that increased continuity. This position typically operates from 8:00 AM to 5:00 PM Monday-Friday (with some flexibility of hours to best serve our patients and support clinic efficiency) with at least one evening (4-8pm) shift a week and occasional Saturday clinic coverage. The clinic is staffed at all times by attending and resident physicians, with ready access to all ancillary services. Attending providers will supervise all care provided by nurse practitioners. The Division has a culture of quality improvement as we serve diverse populations. Subsequently, practitioners will participate in and/or lead practice improvement clinic-based projects.
