As an ECM Lead Case Manager, you will play a key role in the Homeless Healthcare Collaborative team by promoting greater health equity and clinical outcomes for people experiencing homelessness. Primary duties will include:
•Provide enhanced care management by assisting unhoused clients in navigating systems of care and accessing resources that address social determinants of health.
•Perform outreach and collaborate with a multidisciplinary street medicine care team to recruit, enroll, and provide case management services to qualifying individuals for medical and social services.
•Collaborate with clinicians and community health workers to assess patient needs and provide tailored enhanced case management addressing both clinical health needs and social determinants of health.
•Develop care plans and assist clients with linkages to medical, psychiatric, social, educational, housing, food, and other necessary services.
•Travel to community sites as needed, working alongside program clinical teams, patients, caregivers, community partners, office staff, and other key stakeholders.
•Build awareness of UCLA Health's HHC ECM and homeless healthcare programs while improving the health of the unhoused community.
Salary Range: $63,800.00 - $127,000.00/year
Qualifications
•Minimum of 2 years of relevant experience in a case management, including 1 year of experience working with the unhoused or similar populations.
•Prior experience with population management and/or care coordination, particularly with people experiencing homelessness.
•Required to hold valid driver's license, have a driving record that is in accordance with local policies/procedures, and/or enroll in the California Employer Pull Notice Program.
•BLS from the American Heart Association or American Red Cross.
•Ability to interact diplomatically and empathetically with patients in varying degrees of health and effectively communicate with staff, faculty, and the general public.
•Experience with Whole-Person Care or Enhanced Care Management (ECM) work - preferred.
•Experience in street medicine or community-based care for people experiencing homelessness - preferred.
•Lived experience of homelessness - preferred.
•Bilingual proficiency in English and Spanish - preferred.
