Dartmouth Health Cheshire Medical Center is seeking a compassionate and dedicated Palliative Care Physician to join our multidisciplinary team. This role offers an opportunity to provide high-quality, patient-centered care for individuals with serious illnesses while also contributing to program development in palliative medicine.
Key Responsibilities:
Provide comprehensive palliative care services, including symptom management, goals-of-care discussions, and psychosocial support for patients with serious or life-limiting illnesses in both the inpatient and outpatient settings.
Collaborate with a multidisciplinary team, including nurses and social workers to deliver holistic care.
Engage in community outreach and collaboration with referring providers to enhance access to palliative care.
Qualifications:
MD or DO degree from an accredited medical school.
Board-certified or board-eligible in Hospice and Palliative Medicine by the ABMS or AOA.
Eligible for medical licensure in New Hampshire.
Strong communication skills and ability to navigate complex patient and family discussions.
Commitment to interdisciplinary teamwork, education, and patient advocacy.
