Job Summary
You'll be a key figure in organizing our workflow and operations. Responsibilities include staff management, patient care, and physician relations. Your role involves addressing issues from the clinical team with a systematic approach. One main focus is ensuring our team follows Medicare regulations for patient care.
Essential Functions of the Role
You will oversee nurses and staff providing care to terminally ill patients in the community.
Coordinate with Admission Coordinator and Nurse, and patient's family for timely Hospice admissions.
You'll manage team meetings bi-weekly. Professionals from various fields attend. The goal is to update care plans for patients.
You will handle complex patient and family complaints, investigating concerns and taking action to resolve issues.
Finally, visit nursing facilities, assisted living centers, and memory care units to evaluate staff performance and maintain standards.
Key Success Factors
Effective communication skills are crucial. These skills include self-awareness, inquiry, conflict management, negotiation, advocacy, and listening habits.
It's important to know that resources are limited. Patient care planning must prioritize safety, effectiveness, and efficiency.
You would need to assign tasks and care to the suitable interdisciplinary team members and ensure regular follow-ups.
It's important to be organized, multitask, and address staffing and patient concerns efficiently in healthcare settings.
Staying current with Hospice regulations, especially related to patient care and employee education under Medicaid guidelines, is crucial.

Qualifications
M-F 8-5
Must have 3-5 Years as Hospice RN and 1-3 in leadership role, must have understanding of Hospice operations and be ready to assist in the field as needed
Associate's degree.
Experience: At least (2) two years of relevant experience in the field.
Basic Life Support (BLS) certification is mandatory.
