Summary

The New York Harbor VA Healthcare System is seeking a Nurse Practitioner to join our leadership team at the Manhattan Campus.

Duties

• Overall - the nurse practitioner is assigned to help coordinate clinical care for patients with disease processes that may require EP consultation or procedures - perform device interrogations from all the different companies both in person and remotely - interpret device interrogations - and reprogram devices based on recommendations from the electrophysiology service and cardiology service - and enter data into a national database
• Major duties and responsibilities of the position: Enroll each new implant into the EP device clinic - and ensure all devices are registered
• Coordinate care of EP device patients with other services - including medical teams - surgical teams - and radiology Provide ongoing care coordination and monitoring (via telehealth - telephone - and in-person visits) for EP device patients Monitor data that is yielded from permanently implanted devices in selected patients from remote monitoring and optimal management of devices
• Ensure access to all patients with cardiac rhythm devices to the EP device clinic either remotely or in person
• Prescribe medications in discussion with cardiology - especially associated with events discovered at EP device interrogation
• Daily in-person appointments with 10 to 12 patients Answer and schedule consults with the EP device clinic Daily walk-in opening for patients who may experience any symptoms of EP device malfunctioning or received shocks
• Register all new implants or new patents that arrived at the clinic to the National Surveillance Coordinate care and follow all recommendations of the National surveillance
• Monitor and document the remote transmissions of patients currently enrolled in the National Surveillance Patient education regarding wound and EP device care Once a month program and monitor patients who had implanted devices and are scheduled for MRI Answer and educate other disciplines about patient management when patients with implanted devices have procedures
• Education of nursing staff in post-implant care
• Interfacility coordination of care of the patients with an implanted device Long-term monitor (e.g. - Ziopatch) report uploads into CPRS
• Work with the patient safety team - the National Surveillance - and the manufacturer when there are safety recalls
• Some of the duties include
• Informing the patients about the recall or advisory
• Reprogramming the device following the manufacturer's recommendations
• Monitor the patients accordingly
• Reprogram devices before and after surgery and other procedures
• Coordinate clinical care between the EP physicians and inpatient cardiology services related to EP disease processes
