Job Summary:

JOB SUMMARY
Provides treatment planning, safety planning, case management, and brief targeted intervention for social, emotional and environmental issues that impact the client's recovery process.
Actively collaborates with team (MD's, recovery coaches, nurses) to develop and implement a treatment plan to support the client's recovery process.
Contributes to the plan of care with knowledge of issues related to co-occurring disorders.
Maintains a case load of therapy/case management clients.
