Job Summary The Outreach Worker (ORWD) will serve as an integral member of Health Care for the Homeless enabling services team, to conduct in-reach, outreach, street medicine, and education activities to existing health center patients and non-health center patients within the Homeless community. The ORWD will work directly with patients to assess needs across all aspects of their lives, including economic and social challenges alongside their physical and mental health needs. The ORWD will be the point person for consumers within the HCHP targeted population and will be supported by, a care team associated with each patient in order to help improve patients' total health.
