Job Summary:
We are seeking an RN Care Coordinator in the Medicine Specialties Clinic of South Durham, located in Durham, NCto offer individualized assistance across the care continuum to patients, families, and caregivers to expedite and coordinate care, and address health system barriers. The RN Care Coordinator assumes responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to achieve optimal patient outcomes with a focus on Gender Medicine patients with complex care needs (including multidisciplinary care). The RN Care Coordinator has a particular focus on improving transitions in care and adherence to care. Additionally, the RN Care Coordinator functions as a clinical advocate and educator for patients. People with lived experience as transgender or gender diverse are encouraged to apply.
Job Duties and Responsibilities:
First point of contact for people interested registering and scheduling new patients. Educating them on questions regarding first appt, sending email with appt date/time/location and questionnaires and information regarding first visit. Triage patients to ensure they are scheduled with the appropriate specialty for their visits and have all required materials for their visits. This includes assessing eligibility for surgical visits and directing patients to the medicine clinics if not currently meeting surgical eligibility criteria.
Manage intake surveys ensuring completion and entering into Epic. Will also act as member of the clinic's research team. Manage registry research invitation, following up on any inquiries about the study and facilitating completion of research consent. Entering consented patients into ONCORE.
Coordination of services with other specialties (ex: dermatology, surgery, ENT). This includes additional clinic offerings such as the biannual law school event for assistance with legal documentation changes.
Obtain and scan in records for new patients for first appt.
Scheduling return appts for gender clinic if not done at check out or cancel/reschedule when patients call in.
Managing patient waitlist and facilitate scheduling when patients need to urgently be seen.
Triaging emails/phone calls from new and return patients and pharmacies regarding questions about meds.
Manage and coordinate lab orders and results when labs are done outside of Duke.
Provide follow up resources for patients on surgical requirements, community resources, mental health services, surgeon-specific requested services, general and surgical financial assistance services. Assist with the development and distribution of patient education materials.
Create, manage, and disseminate quarterly newsletter regarding clinic updates.
Organize and lead the clinic's presence at community outreach events such as the annual Durham PRIDE.
Perform other related duties incidental to the work described herein.
Job Eligibility Requirements:
Work requires graduation from an accredited BSN program.
Three years of RN experience of specialty specific clinical position in a hospital, clinic, or community setting.
Experience in nurse navigation preferred.
Completion of national nurse navigation training preferred.
Metric tracking skills
Must have current or compact RN licensure in the state of North Carolina. BLS required.
Job Schedule:
The schedule for this position is day shifts, Monday-Friday, with no weekends or holidays.
