Clinica Family Health & Wellness (CFHW), a nationally recognized FQHC, has an opening for a Chief Medical Officer, headquartered in Lafayette, Colorado in eastern Boulder County. Reporting to the Chief Executive Officer, you will be a member of the executive team within our large integrated healthcare organization. This role provides leadership, direction, and administration of primary care medical activities to ensure accomplishment of Clinica’s objectives to optimize patient and population health outcomes. As a practicing primary care provider, you will work within an integrated care team, caring for patients in English and Spanish language.
You will support leadership oversight of the primary care medical team, including direct oversight of medical provider teams and collaborative oversite of clinical care provided by nursing, medical assistants, and other care team members, in partnership with an integrated leadership team. This role shares responsibility for clinical quality improvement and care model design in partnership with other members of the executive and leadership teams, focused on achieving the Quadruple Aim and successfully supporting value-based payment contracts with major insurance payors. You will bring your passion for team-based care, population health, and continuous quality improvement.
CFHW recently completed a merger bringing together the FQHC and partner Community Mental Health Center (formerly Mental Health Partners). Our ideal candidate is excited to collaborate to advance our vision of whole person care and integration of physical health, behavioral health, oral health, and social determinants of health. Other responsibilities include support for peer review processes, relevant policies, protocols and procedures, clinical risk management, infection control, employee health, and laboratory services. The role also supports maintenance and renewal of PCMH and other relevant accreditations including AAAHC, NCQA, and HRSA OSV. In partnership with the CEO, you will attend monthly board of director meetings, supporting clinical reporting and education. You will be an active and visible member of the health care community, representing CFHW and contributing to a variety of venues.

Job Requirements Include:
MD/DO, NP, or PA with board certification or appropriate training and/or experience in family medicine or other primary care specialties.
Valid license/DEA in the jurisdiction of the clinic.
Minimum of five years' progressive experience in medical management and leadership, preferably in a community health center
