Team Blue: W. Longwood, Lake Mary, Apopka, Mt Dora area
The role you’ll contribute:
The Home Health Occupational Therapist (OT), under the general supervision of the Care Manager, provides occupational therapy to assigned patients and directs therapy assistants and Home Health Aides to provide quality care in accordance with physician orders under a plan of care. Assesses, plans, implements, evaluates total patient care and coordinates patient/family teaching. Assures compliance with therapy standards, federal, state, and local government regulations, and company clinical policies. Provides goal directed care leading to specific outcomes. May be on call for after-hours work. Coordinates and directs the home care patient's care based on individual patient needs.

Performs an evaluation, assessing function using a method which objectively measures activities of daily living such as, but not limited to, eating, swallowing, bathing, dressing, toileting, walking, climbing stairs, using assistive devices and mental and cognitive factors, documenting the measurement results in the clinical record.
Administers treatments as discussed in the plan of care involving application of physical agents, and manual techniques to improve and maintain function. Instructs, motivates, and assists the patient to perform various functional physical activities, activities in daily living, self- care activities, and exercises.
Measures and monitors effects of treatment at various stages and confers with occupational therapist to adjust treatments to achieve maximum benefit and outcome.
Assures that patient treatment outcome is directed towards development and restoration of functional skills and returning to or maintaining maximum potential.
Recommends and teaches the use of assistive devices and equipment to patients and families.
Instructs nurses, Home Health Aides, and family members to provide for continuous Occupational Therapy program.
Observes records and reports on patient progress, reaction to treatment and any changes in patient's condition.
Reports critical changes directly to Care Manager and/or other members of the interdisciplinary team
Provides accurate and current documentation.
Qualifications
The expertise and experiences you’ll need to succeed:
EDUCATION AND EXPERIENCE REQUIRED:
Minimum of six-months relevant occupational therapy experience
Cardio Pulmonary Resuscitation
Driver's License
Occupational Therapist
EDUCATION AND EXPERIENCE PREFERRED:
Recent, relevant experience in a Medicare-certified home health agency as an Occupational Therapist
Masters degree in Occupational Therapy
