Shift Information: PT Shift (8am - 4:30pm Mon/Wed/Fri and a half day every Tuesday morning) - No Weekend/Holidays
Home Office Location: - 670 Mason Ridge Center Dr Ste 300 -Saint Louis, Missouri 63141 – You will cover the St Charles area primarily and you may float to greater STL metro area when needed
Case Load: You will be the only social worker for the St Charles area
Job Responsibilities: Meeting with patients/families, determine resource needs, determine social support needs, counseling support needs, veteran benefit eligibility, etc.
Preferred skills or required work experience: home health experience is not required, but definitely a bonus. Additionally, this is a very independent position, prior social work experience is preferrable.
Fully Licensed LCSW is preferred but will consider an LMSW provisional candidate as well.
If provisional LMSW; you must be willing to complete your LCSW (you will be responsible for your clinical supervision)

Role Purpose
Under direction of a physician, assists patients and their families in their residences to adapt and plan an interdisciplinary approach to treating the patient-family system.
Responsibilities
Consults with patients, families and staff regarding goals of care, end of life issues and palliative needs.
Provides direct clinical services to patients and families.
Plans and implements social work services on assigned clinical service or unit in collaboration with the healthcare team.
Collaborates with community human service agencies to meet the needs of patients, families, and the hospital.
