The LVN Case Manager Liaison, in partnership with the Medical Management team and Physician Leadership, will perform case management, coordination of care and medical management consulting within an interdisciplinary team. This includes collaborating with acute and post-acute network facilities, vendors, and Optum partners on complex patient care coordination.
Position in this function provides health education, coaching and treatment decision support for members.

Required Qualifications:
- Current unrestricted NY LVN license
- 2+ years of experience in a physician’s office, clinical or hospital setting
- Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
- Proficient computer skills to work efficiently with electronic medical records
- Ability to travel within a commutable distance of Poughkeepsie for internal and external meetings
Preferred Qualifications:
- Experience related to patient education and/or motivational interviewing skills and self-management goal setting
- Case management experience
- Comfortable speaking and presenting to hospital partners and executive leadership
New York Residents Only: The hourly range for this role is $19.47 to $38.08 per hour.
