The role you’ll contribute:
Responsible to ensure that all referrals all entered into current software system after receiving intake, pulling appropriate documents from Cerner, Epic, or from the physician's office etc. Professional communication with: case managers, home care coordinators, physician's office regarding taking referrals and follow-up phone calls to inform of acceptance of referral. Perform insurance verification to assure that there is not another home care agency involved. Once patient is entered and approved for service scan all appropriate documents into the system. Assist with answering incoming phone calls and distribute to the appropriate personnel. Attends meetings including but not limited to; staff meetings, in-services, case conferences, educational classes, office meetings and/or organizational meetings. Continued employment with the organization is contingent upon your ability to perform job duties in this job description.

Reviews documentation for accuracy and completeness and ensures compliance with Medicare requirements for home health services
Enters referrals into current software system after receiving intake.

Required:
Licensed Practical Nurse in the State of Florida
Minimum qualifications:
Knowledge of EPIC
Knowledge of HomeCare
Knowledge of referral sources
