SUMMARY OF POSITION:

Hybrid with potential for in office work a couple times per month, especially during initial training.

The Clinical Pharmacist at Penn State Health is responsible for providing the evaluation and monitoring of drug therapy and make patient specific recommendations to improve pharmacotherapy outcomes for attributed patients. This position will work within a collaborative relationship with patients, providers and payors to provide patients with an enhanced quality of care. 

 MINIMUM QUALIFICATION(S):

• Graduate of an accredited School of Pharmacy
• Doctor of Pharmacy Degree preferred
• Licensure or eligibility for licensure in Pennsylvania
• Experience with Quality Outcomes (STARS measures, HEDIS metrics etc.) 

PREFERRED QUALIFICATION(S):

• Experience with electronic health records EHR, population health platforms, pharmacy analytics tools, and Microsoft Excel
• Knowledge of Medicare, Medicaid, and commercial value-based care payment models
• Excellent communication, process improvement, problem solving, critical thinking, and leadership skills
