Full-time, Float and Per diem opportunities for any schedule.
We are currently seeking to fill the following full-time, float and per diem Case Manager schedules at Dartmouth Hitchcock Medical Center in Lebanon, NH.
PLEASE NOTE: all full-time positions are 5 days/week, onsite in Lebanon.
Full-time, 40 hours/week positions in Medicine & Cardiology Services
Description of shifts is below.
Monday-Friday work schedule with some flexibility in daily hours worked – 5 (8s).
Weekend call is required two (2) days out of every quarter – can be on the same weekend or broken over two weekends. You will be off call by 8:30am on your call day if no call outs received (so no need to remain available all day). If we identify a staffing need in advance and “plan” to call you in for a call day, you are given the option to take an alternative day off vs. overtime pay.

Dartmouth Health Children’s/ Birthing Pavilion/ Intensive Care Nursery position – Per diem
The RN Case Manager triages, coordinates and facilitates the clinical management for an assigned hospitalized patient population. Develops, monitors and reinforces care standards and protocols designed to enhance quality and promote cost-effective utilization of healthcare resources.
The emphasis is on care coordination and discharge planning, collaboratively working with patients, their support systems, and the interdisciplinary team to achieve the goal of providing the best care in the best place at the right time using evidence-based practice.
Monitors the progress towards the goals of the plan and alerts the health care team to potential revisions to the plan in response to the changes in the patient needs and condition.
Triages telephone calls and responds to patients and families and other members of the healthcare teams questions that are within a registered nurse’s scope of practice.
Communicates information to the health care team and those involved in the treatment and patient’s care.
Recognizes situations that require referrals to Quality, Risk Management, or Patient and Family Relations and discusses with leadership.
Evaluates the options available and balances cost and quality to assure the optimal clinical and financial outcomes. Facilitates progression to appropriate levels of care.
Evaluates patient outcomes affected by the care management process for improvement opportunities. Identifies potential for and participates in quality improvement initiatives within the clinical setting or other organizational committees.
Works with the clinical research to coordinate care for patients on complex protocols.
Qualifications:
Graduate from an accredited Nursing Program required.
Bachelor’s degree (or matriculated into a Bachelor’s degree program with completion within a period of time agreed upon with the department at the time of hire) with 3 years of clinical experience required.
Recent acute care hospital experience preferred.
Required Licensure/Certifications:
Licensed Registered Nurse with NH eligibility
Basic Life Support (BLS) Certificate required.
