Job Summary
Works collaboratively with the IDT members and Patient Care Managers (PCM) to provide coordination of patient care. Answer calls from patients and families. Ensure that calls are transferred to approprate staff and patient/family needs are met. Performs chart audits under the direction of the Clinical Quality Performance Coordinator or Patient Care Manager in timely manner to ensure efficient and effective patient care. Coordination of the daily electronic scheduling and completion of audits in the HAA scheduler's absence. Collaborate with primary nursing staff, PCM and other IDT members regarding continuous care and will make LPN assignments. Demonstrate good communication skills and clinical judgment with patients/family and staff. Will be responsible for accurately maintaining the master schedule for all clinical staff to adequately meet the needs of patient census as outlined in scheduling guidelines. Enter orders into the EMR within the framework of licensure requirements.
Minimum Job Qualifications
Licensure or other certifications: Georgia LPN Licensure.
Educational Requirements: High School Diploma or GED. Current BLS certification.
Minimum Experience: Two (2) years of healthcare experience.

Essential Tasks and Responsibilities
Responsible for accurately maintaining the master schedule for clinical staff. Arrange coverage for all clinical staffing needs. Update the clinical master schedule on a daily basis. Communicate effectively with clinical staff regarding scheduling for coverage of patient care needs. Communicate effectively with supervisor and provide updates of scheduling status daily. Maintain the webxchange on-call schedule for the afterhours operator for on-call purposes. Perform other related job duties or functions as requested or assigned.
Supports a collaborative practice environment utilizing a team approach to ensure coordination of services and enhance continuity of patient care. Actively supports the Patient Care Manager. Provides nursing care to patients within the scope of his or her practice as a licensed LPN. Respond timely and politely to telephone calls and secure chat. Takes accurate messages and triage calls as appropriate. Follow up on calls as needed. Collaborate with Nursing team, Patient Care Supervisors and the Patient Care Manager to coordinate patient care. Provides documentation of care provided through charting in the electronic medical record. Report directly to the Patient Care Manager or Patient Care Supervisor regarding documentation issues and any changes that must be made to the plan of care to meet these requirements.
Actively supports a customer service oriented environment to continually enhance customer satisfaction. Perform phone surveys to admitted patients within the first 2 weeks of admission and then randomly throughout the admission, ensure patient and family satisfaction with hospice services and report any dissatisfaction to the Patient Care Manager immediately. Be a source of reference along with the staff education supervisor to answer questions regarding patient care for the LPN/Aide staff. Assist with the collection of data from various sources to enhance clinical decisions for the implementation of the interventions identified in the plan of care and evaluates response to interventions in relation to expected outcomes. Provide education and instruction to patient and/or caregivers appropriate to the learning ability/level of the patient and/or caregivers and document this education in the patient's plan of care. Handle calls with excellent customer service, compassion and clinical and interpersonal expertise. Demonstrate good judgment and critical thinking skills in response to calls. Inspires confidence from callers and co-workers, remaining well informed and prepared at all times. Demonstrates caring and maintains calm and effective behavior during stressful situations.
Documents care in accordance with medical documentation standards and in accordance with agency policy, standards and timelines. Complete patient charting in accordance with the organization's policy and procedure and within specified timeframes 100% of the time. Charts concisely and legibly. Does not utilized unacceptable abbreviations. Document in a manner that facilitates the determination of expected outcomes and plan of care, including intervention and response to interventions. Complete necessary paperwork/procedures for the appropriate payor to ensure proper reimbursement in accordance with the organization's policy and procedures.
Collaborates and communicates with the patient/family, Primary Nurse, Patient Care Manager, Physician, Referral Source, and Hospice Interdisciplinary Team to ensure optimal coordination of care for each patient/family and to participate in the development and monitoring of the plan of care to ensure effective patient case management. Acknowledge and honor the patient, caregivers, and significant others as vital members of the health care team. Communicates accordingly in assessing and establishing their needs and goals. Delivers care in a manner that preserves and protects patient autonomy, dignity and rights. Consult with Attending Physician/Medical Director, Primary Nurse and other health care providers for patient care, especially difficult to control symptoms of hospice patients. Recommend/assist the Primary Nurse in coordinating all aspects of care for patients and their families by appropriate communications with various IDT members and referrals to appropriate community agencies/resources as indicated. Notifies IDT members of changes in patient's condition/support system/environment. Assist in coordinating changes in level of care and care setting as indicated and in accordance with physician orders and agency policy. Respond appropriately to emergency situations as indicated by the assessment findings and nature of the patient's condition in accordance with the hospice policy and procedures.
Considers factors relating to safety, effectiveness, performance improvement and cost in planning and delivering patient care. Participate in peer review and other Quality Assessment/Performance Improvement activities as directed. Prepare standard reports and assists manager in reviewing reports such as productivity, overtime, visit frequencies, mileage, etc. Document and process all variances, medication errors, adverse drug reactions and infection surveillance in accordance with agency policy and procedure. Evaluate care for a cost effectiveness perspective and utilizing hospice philosophy of care. Understands hospice Medicare/Medicaid/payor benefits and delivers care in accordance with benefit and plan of care. Utilize principles which are based on accepted standards of practice and in accordance with agency policy and procedure to ensure the safety and welfare of all patients.
Maintains responsibility for communication and professional development and growth. Attends interdisciplinary team meetings, staff meetings and educational programs when indicated or obtains information through review of minutes, handout materials, video/audio tape of sessions. Complete all required education within specified time frame. Seeks out education required to maintain clinical and regulatory expertise and knowledge. Complete required competencies within specified time frames. Observe established dress guidelines, visibly wearing the photo/name identification badge. Demonstrate knowledge and compliance with OSHA regulations, organization's safety policies and risk management program.
