The role you’ll contribute:
Care Coordination serves to assist individuals who are not effectively connected with the services and supports the need to transition successfully from higher levels of care to effective community-based care. This includes services and supports that affect a person’s overall well-being, such as primary physical health care, housing, and social connectedness. Care Coordination connects systems including behavioral health, primary care, peer and natural supports, housing, education, vocation, and the justice systems. It is time-limited, with a heavy concentration on educating and empowering the person served and provides a single point of contact until a person is adequately connected to the care that meets their needs.
The short-term goals of implementing Care Coordination are to:
• Improve transitions from acute and restrictive to less restrictive community-based levels of care,
• Increase diversions from state mental health treatment facility admissions,
• Decrease avoidable hospitalizations, inpatient care, incarcerations, and homelessness, and
• Focus on an individual’s wellness, physical health, and community integration.
The long-term goals of implementing Care Coordination are to:
• Shift from an acute care model of care to a recovery model of well-being, and
• Offer an array of services and supports to meet an individual’s chosen pathway to recovery.
The value you’ll bring to the team:
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES:
•Connect with consumers, community organizations, schools, and/or healthcare providers to connect children and adolescents with appropriate services and resources
•Completes psycho-social assessments on patients coping with mental health, developmental/behavioral disorder and substance use disorder.
•Coordinates care plans and support for patients who require multiple services at a time
•Follows patients in post-acute environment to ensure compliance with recovery, medical and behavioral health goals
•Maintains comprehensive community resources and networks to assist patients and families as they transition back to the community
•Makes referrals to federal, state, county, and community agencies in a timely manner.
•Serves as a consultant and educator to consumers, community organizations, schools and or multidisciplinary healthcare team regarding psycho-social components of illness.
Qualifications
The expertise and experiences you’ll need to succeed:
EDUCATION AND EXPERIENCE REQUIRED:
•Two years’ experience in social work or behavioral health
•Master’s Degree in Social Work or a behavioral health related field.
EDUCATION AND EXPERIENCE PREFERRED:
•Experience in acute care hospital
•Experience working with medically complex patients
•Experience working with substance use and mental health disorders
•Experience with school and community settings
