About the Role:
Job Summary
The Coordinator - Health Outreach will be an additional contact between our Care Management Programs and identified members as part of a multidisciplinary care team. The HOC reinforces members health and disease education and empower them to actively participate in their care. The HOC provide telephonic health promotion, and outreach while coordinating Primary Care Provider (PCP) appointments and reminders. The HOC collaborates with other professionals to support on-going needs of identified members. This position is responsible for telephonic outreach to patients. At times this position will function in a hybrid role (virtually/in-person).
Minimum Job Qualifications
Licensure or other certifications: Certified Medical Assistant or Licensed Practical Nurse.
Educational Requirements: High School Diploma or GED.
Minimum Experience: Five (5) years experience in a clinical healthcare setting or customer service and Two (2) years of experience in a primary care setting required. Strong professional verbal skills.

Experience in working with chronic disease states

Adept at learning and using computer systems
Proficient in Microsoft Word and Excel

Essential Tasks and Responsibilities
Reach out to members and engage them in a discussion around case management program benefits and enrollment.
Maintain daily calling schedule.
Facilitate enrollment of member into identified case management programs (i.e., Transitional Care Program, Complex Case Management)
Outreach to low-risk patients during care transitions
Update and maintain notes in business operating system.
Coordinate calls between member and primary care physician office staff to schedule patient appointments and coordinate other activities related to the member's prescribed treatment plan.
Document and track activities in a business operating system.
Coordinate and complete special projects as assigned.
Interact professionally, efficiently and effectively with patients, providers and others of the multi-disciplinary team.
