ABOUT THE OPPORTUNITY:
Under general supervision of the Director of Clinical Support Services the Triage RN performs care coordination and professional nursing functions to assess the severity of the patient?s health concerns using protocols and algorithms in a manner that provides patient dispositions to emergency care, urgent care and/or clinic provider appointments within the scope of the licensed practice. The Triage RN will support providers in triaging patient phone calls and portal messages. The Triage RN will be part of a care coordination/clinical team supporting general care processes alongside the providers
This position is Part Time working 32 hours per week. (4-8hour shifts). Benefits eligible.
WHAT YOU WILL DO:
1. Evaluates and assesses patient?s health care needs based on patient?s signs and symptoms utilizing established triage protocols and guidelines.
2. Participates in weekly huddles to review Key Performance Indicators (KPI)
3. Ensured workflows are within KPI expectations (including call time limit)
4. Obtains appropriate input and direction from the provider as needed to determine best clinical course of action and recommendations for specific patient health care needs.
6. Follows policies, procedures, and protocols to ensure consistency and departmental effectiveness as well as improve health care outcomes of patients/callers and their access to appropriate health care.
7. Assists in quality improvement of program when requested by leadership.
8. Coordinate and schedule same-day and future appointments on the provider schedules
9. Utilize standing orders, triage protocols and algorithms appropriately. ? Manage prescription refills per protocol.
10. Participate in CPC+ Care Coordination team process.
11. Provide patient education: e.g. chronic disease, preventive and family.
12. Participates in Performance Improvement activities as assigned.
13. Assures that pertinent medical information is forwarded to proper departments as needed for continuity of patient care.
14. Documents in electronic health record (EHR), triage assessment, plan, implementation, and evaluation, follow-up.
15. Role models the principals of a Just Culture and Organizational Values.
16. Perform other duties as assigned. Must be HIPAA compliant.

WHAT YOU WILL NEED:
Experience:
Minimum of 3 years clinical experience with applicable patient care and administrative duties preferred.
Must have knowledge of disease processes, normal growth and development of all age groups.
Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations.
License(s):
State of Colorado Registered Nurse or valid compact state license required
Certification(s):
Basic Life Support (BLS) required
Computer / Typing:
Must possess, or be able to obtain within 90 days, the computers skills necessary to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc.
Must have working knowledge of the English language, including reading, writing, and speaking English.
