Senior Focused Nurse Practitioner / Physician Assistant Opportunity in St. Louis, MO!

Mercy Clinic Primary Care-St.Peters is recruiting Advanced Practice Providers with an interest in building relationships, caring for patients with complex needs, changing lives, and making a difference for seniors. This is an outpatient-only position with patients on Medicare and Medicare Advantage.

• Patient panel will offer intellectually stimulating and rewarding environment
• Dedicated panel nurse for telephone encounters, triage, and in basket support 
• In-person, virtual, or telephonic encounters - annual comprehensive visits, transitional care, and same day visits 
• Exceptional work/life balance
• Provide team-based care to manage a panel of 1,000 with collaborating physician allowing for longitudinal relationships with providers, patients and families
• No holidays, weekends, or evenings
