The role you’ll contribute:
The Community Health Worker I (CHW I) is a frontline public health worker who is a trusted member of and/or has a close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. The Community Health Worker I also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. The CHW I will build and maintain a panel of patients in collaboration with the department(s) they support.
The value you’ll bring to the team:
Cultural mediation among individuals, communities, and health and social service systems (e.g., educating individuals and communities about how to use health and social service systems, building health literacy, and cross-cultural communication).
Provides culturally appropriate health education and information (e.g., conducts health promotion and disease prevention education matching linguistic and cultural needs of participants and/or communities).
Care coordination, case management, and system navigation (e.g., makes referrals and provides follow-up, facilitates transportation services and helps to address other barriers to services; documents and tracks individual and population level data).
Under the supervision of professional staff, reviews and selects appropriate health education materials; provides group education on specific health-related topics (e.g., nutrition or communicable diseases), and works on special health education projects.
Assists in creating a positive and supportive work environment; enforces a safe workplace; establishes a culture of teamwork and communication; creates a workplace that promotes the organizational values of workplace diversity, equity and inclusion and actively promotes an environment respectful of living and working in a multicultural society.
Qualifications
EDUCATION AND EXPERIENCE REQUIRED:
Possession of a high school diploma or equivalent
EDUCATION AND EXPERIENCE REQUIRED:
1 year of healthcare experience
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Community Health Worker (CHW) certification in Kentucky
