This position is responsible for the coordination of care for patients admitted to an acute care setting under the care of a WellMed Hospitalists Provider. The Patient Navigator (PN) is the advocate/liaison between patients and their care team to help navigate the patient transition from the acute care setting to their home setting. The PN functions as a dedicated patient-care facilitator without clinical responsibilities. The PN will work collaboratively with the One Partners, to include Primary Care Physician, Care Management, Quality Department, and/or Network Management to facilitate a seamless transition. The position must maintain accurate records, complete a detailed tracker of activities, and develop processes for securing effective and efficient coordination amongst the interdisciplinary team. This position will be under the general guidance of the Hospitalist Program Administrator.
Primary Responsibilities:
Utilize available data and resources to accurately identify members who are in the inpatient setting
Assist Hospitalists team to obtain medical records from outside entities including accurate medication list, specialist consults, hospital records, and any other records requested
Prior to discharge from hospital facility and in collaboration with Care Management and Clinic Health Care Coordinator/Manager, obtain transition appointments with primary care physician, and specialists as applicable; and communicate with patient and/or family transition appointment information
Maintain effective communication pathway with Facility and WellMed Care Management teams to facilitate seamless transition processes
Utilize current electronic medical records and provider portal to reconcile patient information needed to assist with meeting current quality metrics
Participate in daily huddles with Hospitalists, Care Management, and other One Team Partners to discuss patient’s discharge plans, identify any potential barriers, and implement appropriate solutions to address such barriers to mitigate readmissions
Participate in weekly patient care conference meetings and collaborate with the interdisciplinary team, to include Primary Care Physicians, Care Management, and Clinic Health Care Coordinators/Managers to discuss identified barriers and verify appropriate referrals for wrap around WellMed or Community services are secured
Assist with completion of patient satisfaction surveys to evaluate current Hospitalists Program and complete data entry into the appropriate database
Work with the Program and local leadership to execute CVO and Quality strategies
Utilizing current provider portal and other available resources, assist with monitoring quality measures and report gaps to Hospitalists providers to assist with closing of any open gaps prior to patient’s discharge
Performs other related duties as assigned

1+ years of experience as LVN in a clinical, hospital, or medical setting
2+ years of administrative support experience
Ability to work independently, with some supervision and direction from manager
Possesses medical terminology knowledge base
Demonstrated excellent organizational skills, customer service skills, and verbal and written communication skills to include but not limited to patients, physicians, clinical staff, contracted providers and managers
Ability to maintain and demonstrate a high degree of professionalism to include both personal conduct and appearance at all times
Ability to maintain strict confidentiality at all times to include but limited to patients and coworkers.
Ability to comply with all organizational policies regarding ethical business practices
Ability to adhere to all department policies and procedures
Proven basic knowledge of Microsoft Office products, including Word and Outlook
Preferred Qualifications:
2+ years of experience in a physician’s clinic or hospital
Experience working in a medical care setting as a receptionist or Medical Assistant
Bilingual language proficiency (English/Spanish)
