As a Case Manager at CommonSpirit Health, you'll advocate for patients while collaborating closely with interdisciplinary health care partners, facilities and families to ensure the best possible care experience from admission through discharge and beyond by connecting patients to resources that help them maintain and build on positive outcomes.
Utilize clinical expertise, discretion, and independent judgment in assessing/reassessing, facilitating care coordination, utilization management, and patient advocacy.
Assure medical appropriateness criteria are met for status and level of care.
Schedule: up to 16 hours per pay period (2 weeks), 08:00 AM - 04:30 PM, per request needed
Qualifications:
In addition to bringing your whole self to the workplace each day, qualified RN Case Managers will need the following:
Nursing Diploma or Associates Degree in Nursing required; BSN preferred
RN License in Applicable State of Employment required
1 year of nursing or case management experience required.
Relevant clinical experience
Clinical experience in acute care settings
Familiarity with healthcare regulations and best practices
Working knowledge of regulatory requirements and accreditation standards strongly preferred.
Proficiency in electronic health records (EHR) and other healthcare technologies
Ability to work well in a multidisciplinary team
Demonstrated critical thinking and decision-making abilities
Flexibility and adaptability to fast-paced and dynamic work environments
Strong communication and interpersonal skills
1 year of nursing or case management experience required.
BLS from the American Heart Association preferred
Physical Requirements - Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
